Country Villa and HealthMEDX Partner to Deliver the First EMR Version of the AHRQ On-Time Avoidable Nursing Home Transfer Module

Goal to decrease avoidable transfers and readmissions

October 12, 2012- OZARK, MO: Country Villa Health Services and HealthMEDX are joining forces to automate the AHRQ On-Time Avoidable Nursing Home (NH) Transfer Module. This new AHRQ Module, which incorporates principles of INTERACTII, is designed to decrease avoidable transfers and readmissions in long-term, post-acute care settings (LTPAC).

The On-Time Avoidable Nursing Home (NH) Transfer Module provides clinical decision support tools and implementation strategies that integrate clinical evidence into workflow processes and proactively identify residents who are at potential risk for readmission.  The complete module streamlines and standardizes transfer and admission documentation incorporating clinical best practices to deliver timely decision-support tools, in the form of reports, to caregivers.  The HealthMEDX Vision EMR automates the workflow and sharing of information and alerts between LTPAC providers, physicians and health systems. 

Country Villa, a 45 facility long-term and post-acute care provider in California, will be the first organization to implement the AHRQ module enabled through HealthMEDX.  The HealthMEDX Vision EMR will provide the AHRQ module’s clinical decision support tools and real-time integrated access to all clinical information and recommendations.  Also, the HealthMEDX Vision EMR will allow physicians to sign orders, review resident charts, and communicate electronically with members of the care team via their iPad® or iPhone®.

The AHRQ On-Time Avoidable NH Transfer Module is one of four On-Time modules developed by AHRQ under contract to Health Management Strategies, Inc. and the Institute of Clinical Outcomes Research with additional support from California HealthCare Foundation. The design of the program, implementation process, and the facilitation of facility teams are provided by Siobhan Sharkey, MBA, and Sandy Hudak, MS, RN, Principals at Health Management Strategies, Inc.
